New patients: Please use the contact form below to request an appointment or for more information. Our office hours are 8:30 a.m. to 5:30 p.m. Monday through Thursday and 8:30 a.m. until 1 p.m. on Fridays. We also are open for appointments in Gainesville and Braselton on Saturdays from 8:30 to noon. Please note that we make every effort to respond to form submissions within one full business day (not including weekends).
Current patients: Please call us reschedule appointments or for prescription refills. We cannot respond to those requests via contact form. If you have an urgent medical concern that is not an emergency and need to speak to a provider on call, please dial one of our office numbers above and follow the prompts. If your child is experiencing a medical emergency, dial 9-1-1 or go to the closest Emergency Room.
